Here you will answer any two of five short answer questions in your own words. A few sentences is all that is needed.

Assessing appearances is an important part of making good matches. We therefore require at least one current photograph of you prior to your telephone interview. You can upload this photo directly, e-mail it to us as an attachment, or mail a hard copy to us so that we can scan it into our system and return it to you.

You will control the distribution of your photograph. We will distribute it only with your permission.
